Logicoo Form turns complex forms into simple multi-step experiences. This free plugin combines a visual three-column builder with conditional logic, live calculations, a review summary, submission limits and built-in statistics. Forms are stored in your own database and work on any device and browser, in most cases even with JavaScript disabled. Build quote requests, registrations, surveys and order forms without touching code, and keep full control of your data.
Build without code
Build multi-step forms in a visual three-column editor. Split fields into pages, add a progress bar and use AND/OR conditional logic to show or hide questions based on earlier answers. Choose from 19 field types, from text and dates to ratings, file uploads and digital signatures. Publish with a shortcode and send several independent email notifications per form, each with its own recipients, conditions and attachments.
Privacy, protection and style
Logicoo Form works with the WordPress tools for exporting and erasing personal data, can skip storing IP addresses, and keeps signatures, uploads and PDFs in a protected folder with no public URL. Protect your forms with reCAPTCHA v2 or v3 and rate limiting. Match your brand with global styles and per-form button texts and colors. The interface is translation-ready, and uninstalling removes all of its data unless you choose to keep it.
Keep control of your data
Every submission is stored in your own WordPress database and managed from one entries screen: filter, view, delete in bulk and export to CSV. A statistics page shows views, submissions and conversion rate for each form. Limit submissions per IP address, session, registered user or in total. Generate a branded PDF for each entry, and remove old entries and their attachments automatically.
Calculate as visitors type
Assign a numeric value to each option and let a Total field add it up live. Link it to a Quantity field, apply quantity-based discounts and show the result in a real-time summary. Before submitting, visitors review all their answers on a final screen and can still edit them. Quotes, bookings and order forms become self-service, and the server rechecks every total on submission for security.

Works without JavaScript
